Child Pre-Registration Form

For Spanish (Español), Arabic, Turkish (Türkçe), Haitian Creole (Kreyòl Ayisyen), or some other language:

We’re excited to support you and your children!

To get started, we need to determine if you’re eligible for our Head Start or Early Head Start early learning and child care programs.

What sets us apart is our two-generational approach that builds family well-being by simultaneously working with children and the adults in their lives to access new resources, solve problems and sharpen existing skills. We have Family Support Specialists, Home Visitors and Coaches ready to partner with you on this journey.

In most cases, families must earn 100% of the Federal Poverty Level or less. Children experiencing homelessness or who are in foster or kinship care also are eligible. In some cases, children with disabilities may qualify.

Don’t assume your family doesn’t qualify! You may be surprised!

If you qualify, you may continue receiving our services once you’re enrolled EVEN if your income increases until your child is no longer age-eligible for their program.

Complete the short questionnaire below, and we’ll notify you about your eligibility via email and text. If you don’t hear from us within 5 business days, please call 937-2EN-ROLL [937-236-7655].

Housing Situation

If a person does not have permanent housing (who may be living on the streets, staying in a shelter, a mission, individual room facilities, an abandoned building or vehicle, or is in any other non-permanent situation) then he or she can qualify as homeless.

It can also be considered that a person has no home if that person is "doubled up", a term that refers to a situation in which people can not maintain their housing situation and are forced to stay with a number of friends and / or members of the extended family.
