To qualify:

- Children must be under the age of 3 or applicant be pregnant

- Families be income or categorically eligible

Income Eligibility = Earn below 100% of the federal income guidelines or receiving SNAP, TANF or SSI, a child in foster care, or experiencing transitional housing.
To apply please complete the fields below and submit the following documentation:

- Proof of age of applicant child

- or Proof of pregnancy (if adult applicant is pregnant).
