Prior to starting the application, please gather the following documents. 1. Proof of Age- Birth Certificate 2. Proof of Household Income - last 2 pay stubs, W2, TANF, SNAP, SSI or Child Support document 3. Parent/Guardian Photo ID 4. Proof of Residence- Lease, utility bill or letter from lease holder if you are residing with someone 5. Child's Immunization Record or Religious exemption letter. 6. Child's Physical Exam 7. Child's Dental Exam 8. Child's Individualized Education Plan (IEP), 9. Custody Papers. If you do not have these documents available you can still submit the application. After you have submitted the application, a confirmation email will be sent to you with a link to upload these documents. A Family Advocate will reach out to you to retrieve any documents you were unable to upload. Please fill out all sections.
