Thank you for your interest in one of the MGCAA Head Start Centers. ALL DOCUMENTS MUST BE SUBMITTED WITH THE APPLICATION IN ORDER FOR IT TO BE PROCESSED. DOCUMENTS MUST BE SUBMITTED ON THE NEXT PAGE AFTER CLICKING SAVE AND CONTINUE. Required documentation are as follows (birth certificate, social security card, updated immunization form, insurance card, and one month's proof of income. If you are unable to fill out the application please contact Ashley Austin at 478-922-4464 ext 1129. If you have no income, please complete the ERSEA 1C. Form FCP 94A is also a requirement for the application and can be located on the website. Based on your eligibility, you will be placed on the waiting list. Once slots are available, you will be contacted for enrollment. MGCAA Head Start Centers requires an email address for families to participate in our program. Don't have an email address? Click Save and Continue 3. Type in the last name of the child and the child's date of birth. 4. Upload all required documents (birth certificate, social security card, updated immunization form, insurance card, and one month's proof of income). Submit the ERSEA 1C located on the website if you have no proof of income. 5. Click the "Submit Application" button at the bottom of the page to submit your application.
