Income qualifying children and families with the greatest need have the highest priority for enrollment, but families with income above the Federal Poverty Guidelines are still able to apply. Please turn in the following documents by emailing copies to hs-apps@caprw.org or faxing to 651-603-5986: Proof of child's birthday (birth certificate, medical records, passport, I-94, etc.), child's immunization record, proof of family income (taxes or paystubs), MFIP/SSI statement, SNAP letter or picture of EBT card, foster care letter, proof of homelessness, and child's most recent physical.
