Intake appointments will be completed with our Family Facilitators and are a requirement for our program. Our facilitators will require parents or guardians to complete and sign our Income and Residency forms as well as our Family Partnership Agreement Information Sheet. These forms help us compile the information needed to determine enrollment eligibility. During this meeting, facilitators will also be able to provide answers to any further questions you have about our programs. We will also need copies of Birth Certificates for the applicants, Immunization Records, and pay stubs or award letters to verify income of the parent or guardian. These verifications can be emailed to bsanders@capwi.org or sent via text message to 765-585-1234.
