Thank you for your interest in the TEAM, Inc. Early Childhood Program. This pre-application is the first step in the enrollment process and allows us to begin gathering basic information about your family and child. After submitting this pre-application, a member of the TEAM staff will contact you to complete the full application and schedule a convenient time for you to provide all required eligibility documentation. Once your child’s application is complete, it will remain active for one year. After that time, updated eligibility documentation will be required to keep the application current. If your family has applied in the past or previously had a child enrolled in TEAM's Early Childhood program, you do not need to complete an online pre-application. Instead, please call the main office to schedule an intake appointment to update your application.
