Lake Cumberland Head Start application is for families living in Adair, Casey, Clinton, Cumberland, Green, McCreary, Pulaski, Russell, Taylor, and Wayne counties who have a child between the ages of 6 weeks and 5 years old. Different program options are offered in each county. Please make sure you are choosing the option you want.
Primary Adult means that you are the child's biological parent and/or legal guardian, including foster parents. If you are applying for the Prenatal program, you are the Primary Adult.
Please indicate if you receive mail somewhere other than the place you live.

If Secondary Adult does NOT live with child, please do not check "lives with family" box. If Secondary Adult does not live with family, but has visitation rights Lake Cumberland Head Start will need a copy of the most recent visitation/custody agreement.

TANF in the state of Kentucky is distributed through the K-TAP program. If your family receives K-TAP or is part of the Kentucky Works program, then your family is considered to receive TANF benefits; therefore, you would answer yes to this question.
Please include any health concerns (e.g. Diagnosed Asthma, Seizures, etc), developmental concerns and/or First Steps involvement in the application notes so that we can provide you with all necessary health paperwork at your intake appointment.
