Already enrolled? If your family is already enrolled in Preschool Promise and you're applying for another child, please call us to complete the application by phone. The online system doesn’t allow returning families to reapply.

This pre-application form collects your basic information. Additional eligibility questions and document uploads will follow in later steps.

Email is Required: Please provide a valid email address for important updates about your application. If you do not have an email, you can create one here.

Please note: An appointment will be required to turn in the paper application and documents. We will also have follow-up questions. See below for our contact information.

You’ll be asked to upload documents in later steps. Here are examples of what you may need:

  • Age verification: Birth certificate, hospital record, immunization record
  • Income verification: 3 recent pay stubs, W-2, benefits letter (SNAP, OHP, etc.)
  • Address verification: Utility bill, lease or rental agreement, Oregon ID

We encourage you to select up to three preschool programs where you would be willing to enroll your child.

Before making your selection, if you haven’t done so already, please visit preschoolhub.org to explore available options. Click on each school to view their location, schedule, and details.

  • Make sure the location and schedule work for your family
  • Check if extended care is available and affordable (if needed)
  • Consider if the teaching style and approach fit your values
  • Call the school to schedule a tour if you want to visit

At the end of this application, you’ll be asked to rank your choices in order of preference. Your first choice will be considered your top program.

Residency Requirements

  • Applicants must have a current Oregon address (a physical address). P.O. Boxes can be used for mailing purposes, but we require a physical address to be filled out for eligibility.
  • If you are moving to Oregon, wait until you have moved or signed a lease before applying.
  • Proof of citizenship is NOT required.
  • If you are experiencing homelessness, PLEASE ANSWER ONLY THE FIRST QUESTION. YOU CAN SKIP THE ADDRESS SECTION, EVEN THOUGH IT MAY STILL SHOW AS REQUIRED.

What counts as homelessness?

A family may be considered homeless (unhoused) if they're experiencing any of the following living situations: in a shelter, in a motel or campground due to the lack of adequate alternative accommodation, in a car, park, abandoned building, or bus/train station, doubled up (staying) with others due to housing loss or economic hardship, lacking a fixed, regular, and adequate nighttime place of residence.

  • The secondary parent or guardian is currently living in the home
  • They are supported by the same income
  • They are related to the child by blood, marriage, or adoption

  • Any developmental delay or disability (physical, developmental, or intellectual)
  • Behavioral or mental health challenges
  • Health or nutrition concerns
  • Health conditions that require specialized supports
  • Challenges your child has experienced in the last year
  • Support from outside agencies like therapy or special programs
  • The name of your child’s service coordinator (if known)

If your child receives special education services, has an Individualized Family Service Plan (IFSP), or is working with Early Intervention (EI) or Early Childhood Special Education (ECSE) to support their development — please let us know.
