NEW PARENTS/FAMILIES: Please read carefully and complete each field. When asked for a location preference, please only select the center located in your county of residence. Center staff will contact you soon to set up a phone interview. You or a household member (or someone authorized to complete the application for your household) must be interviewed by our Family Partner. The individual who is interviewed must know about your household situation. The interview is an official and confidential discussion of the household’s circumstances.

• Head Start/Pre-K (3 year - 4 year old program)

• Referrals (letter from Special Education Department-School District, DFCS, BCW, etc.) - If apply

• SSI (Supplemental Social Security letter) - If apply
