Thank you for completing your application. A Family Service Advocate staff member will be contacting you soon for a follow up appointment. Please have the following documents ready for your follow up appointment: Proof of income which includes proof of Cash Aid or SSI, Unemployment Benefits, Foster Placement Letter OR one month's worth of paystubs for each parent/guardian in the home; AND Birth Certificates for EACH child in the home; Immunization Records for the applying child; MediCal or Insurance Card for the applying child; Court ordered paperwork such as custody papers, restraining orders, etc, if applicable; IEP or IFSP for the applying child, if applicable.
