You will be required to provide: Proof of income (previous years' tax return or W2s, or pay stubs from the last 12 months) Proof of your child's Date of Birth Proof of address, and proof of Foster Care, TANF, SNAP or SSI if applicable at your in-person interview so please begin collecting these documents.
