Applications are currently being accepted for the 2021-2022 program year, which runs August 1, 2021 to July 31, 2022. 1. You will be contacted by a Family Services Specialist within three business days to set up an intake appointment. 2. You must bring the following documents to the intake appointment: • Income verification (W-2, 1040 or pay stubs for 2020) • Up-to-date Well-Child Check • Child's Certificate of Immunization • Medicaid card • SC Voucher letter of approval 3. If the person completing the application is not the parent of the applicant, documents (custody agreements, guardianship/caregiver orders, signed parental statements, foster care contract) giving permission to apply for educational and health services will be required at the intake appointment 4. Applicants are responsible to keep us updated if your contact information changes. We also rely on you to respond to our efforts to reach you. We will generally make 2 attempts to contact you and may need to move to the next applicant if we do not receive a response. 5. If the person completing the application is in school you must bring a copy of your transcript or admission letter at the time of your intake appointment.
