For children that reside in York County Maine Important information: You will not need to complete this pre-sign up if you have applied to our program within the last 5 years. If this is the case, please call 207-459-2965. Your child must be up to date on immunizations in order to participate. Transportation in not available for our Early Head Start and Head Start students. Please make sure you can provide transportation to and from school for your child. This is an online pre-sign up for families living in York County and will take you about 15 minutes to complete. You will need to provide the following information: 1. Full name, birth date, race and primary language of all household members. 2. Highest level of education, phone number and email address for all adult household members. 3. Your child's doctor, dentist and insurance information. 4. Household annual gross income (before taxes) or a copy of current SNAP, TANF, SSI letter.
