Contact your local Head Start/Early Head Start Office through email at application@region10.org and include your child's name, telephone number and contact e-mail address and a Head Start/Early Head Start staff member will get back to you as soon as possible.

*Child’s Birth Certificate,
*Immunization Record,
*Proof of Income (Current: Tax transcript, 1040 with all W-2's, Pell Grant, Child Support and/or current income),
*Proof of Public Assistance/Services (TANF, SNAP, SSI, Medicaid),
*Proof of Residency (Utility bill with name and address)
*Documentation of Child’s Disability/ARD (if applicable)
